I, the undersigned, agree to indemnify and hold harmless Tango Fantástico, Sue Flanagan, Jorge Torres, Dance Boulevard, Ballroom Dance Supply, and their employees, agents, and officers. I release them from any and all liability which may arise as the result of my participation in activities I attend or register into; Furthermore, I assume full responsibility for any and all damages, injuries and/or losses that may occur.

I am aware that I have registered for a class involving physical activity. I have taken care to enroll at a class level appropriate to my physical abilities and/or mental condition.

I release use of my photos and/or video taken during program participation from any and all claims and demands resulting from their use in program publicity.

By submitting this form I understand that I am registering for the class described and I agree to pay the fees for the all classes for which I am registering. I understand that my registration constitutes a commitment to pay for reserved spaces in the designated class and that I must bring my cash payment to class. Class fees are non-refundable.
